The last two comments would be right. If it is very gross to you don't do it. It is your body. I have known a few women who were open to it and one who actually enjoyed it. Most men see it as a final frontier with their woman or their sexual conquests. Okay, first the condom questions. I have never been able to get enough sensation wearing a condom to perform. I am not some player with dozens of partners so I try to be careful with who I am intimate with. If the penis is ridgid enough to enter the anal cavity their is no pain for the penis. Much care must be taken to ensure that the anal cavity is ready. This can take a little time. Usally addtional foreplay involving anal stimulation will help.
